Affidavit of Correction- to be use if title has incorrect markings such as scratch out, white out, incorrect assignment, etc.
Affidavit of Non-Use- to be used if vehicle has been unused or will be unused for any length of time.
Affidavit of One In The Same - to be used if the customer is using any variation of name: Ex: His name on drivers license is Jonathan Brian Moore, but he signed documents as John B Moore, or John Moore.
Power of Attorney - gives another person the authorization to sign on behalf of vehicle owner.
Affidavit of Repossession - to be used when a dealer or lienholder has repossessed at vehicle.
Release of Lien - to be signed by company possessing a lien.
Request for 50 State VIN Check - needed when filing a lien or bonded title.
VIN Ispection Form - must be completed by certified VIN Inspector.
Application for Personalized Vanity Plate - used to order a personalized or "vanity" plate. A regular plate will be issued at time of registration and then the Personalized plate (after mailing the application and fee) will be issued and mailed to the consumer.
Specialty Plates - Many styles of plates are available for most standard vehicles. Most benefit the cause shown on the plate and require a yearly payment of the additional fee.
